Thank you for considering returning to a ministry in the Catholic elementary schools within the Diocese of Brooklyn.  The attached request for reactivation of your teacher file in Adobe Acrobat format is to be downloaded and completed in its entirety.  An incomplete request form will not be processed.

 

We encourage you to read THE INSTRUCTIONS carefully.  Your compliance with all the statements #1 through # 6 will allow us to process your request for reactivation.

 

Please note that there is a twenty-five dollar ($25) non-refundable reactivation processing fee.  Therefore, with your completed reactivation application, kindly enclose a check or money order made payable to: Superintendent of Schools.

 

All applicants must be practicing Catholics; therefore, requests for reactivation must include a signed Pastor's Approval Form from a clergy person in the parish where the individual presently worships before the reactivation request can be finalized.  The Pastor's Approval Form is attached in Adobe format. Please download this form and present it to the Pastor with a stamped envelope addressed to this office.

 

It is essential to write a complete address, including zip code, for all references - Employment History as well as Personal References.  Your reactivation request will be delayed if this information is missing.

 

Requests for Reactivation forms are to be returned to the attention of:

You will be contacted once we have received and reviewed the reactivation request, your Pastors Approval Form and all the appropriate OFFICIAL college transcripts, which you should arrange to send directly to this office if they are not already on file.
